With a real deck, you cannot draw the same card twice until you reshuffle. By default each spin here is a new draw from all 52 cards, so the same card can come up twice. To draw like a real deck, open Customize, choose Winner and set Mode to "Elimination (remove winner)". Each card you draw then leaves the wheel. To get the full deck back, pick "Full deck" in Quick lists.
